The Wincrest Orthodontics Something Nice Grant Program is offered by Wincrest Orthodontics (“Wincrest Orthodontics” or “The Practice”) for patients in need of orthodontic treatment who have budgetary constraints making access to that care a challenge. Once an application (“The Application”) is submitted, the submitter (“The Applicant”) will be considered to receive a grant of up to Two Thousand Dollars ($2,000) to offset the cost of their orthodontic care.

• The Applicant must want to receive orthodontic care at Wincrest Orthodontics.
• The Applicant must demonstrate financial need and be able to afford up to $200 per month for treatment.
• The Applicant must be willing to share a brief personal story or video describing how this grant would help them.

• The grant provides up to $2,000 toward The Applicant’s orthodontic treatment and is non-transferrable.
• The grant cannot be combined with any other discounts or offers.
• Payment plans are available for up to 24 months, regardless of treatment length.

• Complete the Application
  • Fill out the secure online form, including contact details, requested amount, and a personal story or video.
  • Submit the application and look for a confirmation email.

• The Application is reviewed within three business days.
• The Application is approved based on responses to the questions related to budget, orthodontic contracting and compliance, and the impact the grant will have.
• The Applicant will be notified of the decision by phone, email, and/or SMS.

• The Applicant has 30 days to accept the grant and schedule your treatment.
• If The Applicant does not accept or start treatment within 30 days, the grant offer will expire.

All protected health information is maintained in accordance with the Wincrest Orthodontics HIPAA and privacy policy. The Applicant may choose to provide a media release to consent to the use of narrative, visual, and/or video content in marketing and social media promotions. Withholding this consent does not impact the Applicant’s likelihood of being approved for a grant. By completing this application, The Applicant must acknowledge and agree to the following:

• The Applicant is applying to receive a grant towards orthodontic treatment at Wincrest Orthodontics and no other office.
• If the grant application is accepted, the Applicant will be awarded a grant to offset the cost of orthodontic treatment at Wincrest Orthodontics.
• The Applicant understands that, to accept the grant, they will have to sign a contract with Wincrest Orthodontics to begin orthodontic treatment.
• The Applicant understands that there will be a cost associated with orthodontic treatment, even if they Applicant receives this grant. This grant cannot be used in combination with any discounts offered by the practice and is nontransferrable.
